Good Movement Client Liability Waiver

 

I, hereby acknowledge and agree to the following terms and conditions before participating in any activities, classes, or sessions at Good Movement, PLLC .

1. Assumption of Risk:

I understand that participating in Pilates activities involves certain risks and potential hazards. I voluntarily assume all risks associated with my participation, including but not limited to physical exertion, potential for injury, and interaction with other participants or equipment.

2. Health and Fitness:

I declare that I am physically fit and have no medical conditions, injuries, or limitations that would prevent me from participating safely in Pilates classes. If I have any concerns about my health or fitness, I will consult with a medical professional before engaging in any activities at Good Movement.

3. Release of Liability:

I hereby release, waive, discharge, and hold harmless Good Movement, PLLC, its owners, instructors, employees, and contractors from any and all liability, claims, demands, actions, or rights of action that may arise from any injury, harm, loss, or damage to myself or my property during my participation in Pilates activities at Good Movement, PLLC.

4. Responsibility for Personal Belongings:

I understand that Good Movement,PLLC is not responsible for any loss, theft, or damage to personal belongings brought onto the premises. I agree to take full responsibility for my personal belongings and release Good Movement from any liability related to such incidents.

5. Photography and Publicity:

I grant Good Movement, PLLC the right to take and use photographs or videos of me for promotional purposes, including but not limited to website content, social media, and marketing materials. I waive any rights or claims for compensation associated with the use of these materials.

6. Agreement to Follow Studio Policies:

I agree to comply with all rules, policies, and instructions provided by Good Movement, PLLC. I understand that failure to do so may result in my removal from the premises without any entitlement to a refund or compensation.

I have read this liability waiver and fully understand its contents. I acknowledge that I am signing this waiver voluntarily and without any coercion. I understand that by signing this waiver, I am releasing certain legal rights that I or my heirs, executors, administrators, or assigns may have against Good Movement, PLLC.
